Accessing SDS in Angus

If you or the person you care for is eligible for SDS, put in a referral by calling 03452 777 778 or emailing firstcontact@angus.gov.uk.

Someone can put in the referral for you, with your consent.

If you have power of attorney or guardianship duties for someone, you can put in a referral on their behalf.

What happens next

We'll get back to you and ask for more information about your circumstances.

Once we have the information we need, we'll assess whether you meet our eligibility criteria.

If you do not meet the criteria, we can direct you to other sources of help and advice.

Assessment

If you are eligible for support services, an allocated worker/duty worker will assess your needs.

They'll help you identify your own skills and resources, and work with everyone involved to look at ways to improve your life.

We will then identify an individual budget based on your needs.

One tool we use to assess needs is our Adult Self-Assessment form.

You can fill this out before you meet with your allocated worker/duty worker. They will give you a paper copy of the form if you need one. You can ask friends, carers, family and others to help you complete the form.

Financial Assessment

We need to carry out a financial assessment as part of this process. This is because you may be asked to pay a contribution towards your care.

If we have told you that you are eligible for SDS, you can fill in a financial assessment form online.

Support Plan

After the assessment, we will agree a support plan with you.

This plan will outline the actions we will take to achieve the outcomes you want.

It will include details of the resources that will be used, and the individual budget.

You will then discuss with your allocated worker which Self-Directed Support Option you would like.
